Membership is free for qualified medicinal cannabis patients. All applicants for Patient Membership in MFC are required to be medical cannabis patients in the State of California.

If you wish to become an MFC Patient Member, turn in a completed MFC Patient Membership Application and provide copies of the other required documents. A copy of your current physician's medical cannabis authorization and California Driver's License or CA ID must be included in order for us to process your application. Return your application to the MFC by hand, mail to us at: P.O. Box 2129 Laytonville CA 95454, fax to: (707) 984-8701, or email it to us as an attachment.

It is a membership requirement that your status as a qualified medical patient be verified by the MFC with your authorizing physician. We recommend that your patient qualification be verified prior to your first visit to MFC as membership cannot be approved and no medicine can be obtained without your Physician's verification of your medical qualification. You must bring originals of all required documents at the time of your first visit.
